The Appointment Request Form is provided to request a date and time for an appointment.  Your appointment date and time will be confirmed by a Gillis Chiropractic staff member when they contact you by phone.  If you do not receive a phone call from a Gillis Chiropractic staff member 24 hours prior to your date and time requested, please contact the appropriate office to confirm your appointment date and time.
